《中华骨与关节外科杂志》2022年10月第15卷第10期Chin J Bone Joint Surg,VoL.15,No.10,Oct.2022763国家卫生健乘委加速康复外科专家委员会骨科专家组,中国研究型医院学会骨科加速康复专业委员会,中国康复技术转化及促进会骨科加速康复专业委员会】骨科大手术加速康复围手术期营养管理专家共识门中华骨与关节外科杂志,2022,15(10:763767,·专家共识·骨科大手术加速康复围手术期营养管理专家共识国家卫生健康委加速康复外科专家委员会骨科专家组,中国研究型医院学会骨科加速康复专业委员会,中国康复技术转化及促进会骨科加速康复专业委员会【摘要】骨科大手术围手术期营养不良的发生率较高,但其中仅有极少数的患者得到及时的珍断与治疗。营养不良是骨科大手术患者术后并发症的独立危险因素。围手术期合理的营养支持可减少围手术期并发症、加速患者康复,是实现骨科加速康复的关键环节之一。为了进一步加强骨科大手术加速康复国手术期营养管理,提高医疗质量和安全,国家卫生健康委加速康复外科专家委员会骨科专家组、中回研究型医院学会骨科加速康复专业委员会、中回康复技术转化及促进会骨科加速康复专业委员会联合多学科专家成立骨科加速康复系列共识编写专家委员会,以临床问题为导向,遵循循证医学原则,归钠出4个方面的问题:①围手术期营养不良的特征与营养支持目标;②术前营养风险筛查及营养状况评定;③术后营养风险筛查及营养状况评定;④围手术期营养支持。按照上述4个方面的问题进行文献检索和归钠,经过反复讨论和修改后形成本共识,期望其为我回骨科加速康复圆手术期营养管理的多学科协作起到积极的推动作用,最终达到加速患者康复的目的。【关键词】骨科手术:加速康复外科:围手术期:营养管理:营养不良:营养风险:营养风险筛查:营养状况评定【中图分类号】R68【文章编号】2095-9958(2022)10-0763-05【文献标志码】AD010.3969/i.issn.2095-9958.2022.10.07Expert consensus on perioperative nutrition management in major orthopaedic surgeryin enhanced recovery after surgery programOrthopaedic Expert Group of ERAS profession Committee of National Health Commission,Orthopaedic ERAS profession Committeeof Chinese Research Hospital Association,Orthopaedic ERAS profession Committee of China Association of RehabilitationTechnology Transformation and PromotionCorresponding Author:HU Wen,QIU Guixing,PEI Fuxing[Abstract]Orthopaedic patients have a high incidence of malnutrition during perioperative period,and only a small number of suchpatients have been timely diagnosed and treated.Malnutrition in patients undergoing major orthopaedic surgery is an independent riskfactor for postoperative complications.Reasonable perioperative nutritional support can reduce perioperative complications andaccelerate the recovery of patients,which is one of the key links ofenhanced recovery after surgery (ERAS).In order to further promotenutrition management in major orthopaedic surgery in ERAS program and improve medical quality and safety,Orthopaedic ExpertGroup of ERAS profession Committee of National Health Commission,Orthopaedic ERAS profession Committee of Chinese ResearchHospital Association,Orthopaedic ERAS profession Committee of China Association of Rehabilitation Technology Transformation andPromotion,together with experts from other relevant disciplines,established a collaborative expert committee for this consensuswriting.Focusing on the clinical problems and following evidence-based medicine principles,the experts summarized the followingfour aspects:Ocharacteristics of perioperative malnutrition and goals of nutritional support;2preoperative nutritional risk screeningand nutritional status assessment,3postoperative nutritional risk screening and nutritional status assessment;nutrition supportduring perioperative period.Around these four aspects,literature was searched and induced.Through repeated discussions andrevisions,this consensus was developed hoping to promote the multidisciplinary cooperation and realize rapid recovery for patients.[Key words]Orthopaedic Surgery;Enhan
